My wife and I are both central Government employees and have dependent brothers / sisters and parents. Can we make separate cards and include family members?

Where both husband and wife are Central Govt employees either of them may prefer claim for self and eligible members of the family, according to the status of the claimant. 

A joint declaration as to who will prefer the claim should be furnished. If declaration is not furnished, concession is to be availed by all including wife according to the status of the husband. 

If spouse is employed in a State Govt, Defence/ Railways, Corporations or bodies financed partly or wholly by the Central or State Govt local bodies and private organisations which provide medical services, the medical facility can be availed at one place only and a joint declaration as to who shall prefer the claim is required to be furnished. 

In case both husband and wife contribute towards the ECHS Scheme, the parents of both will be entitled to the medical facilities if they fulfill other conditions of eligibility.

जहां पति और पत्नी दोनों केंद्रीय सरकार के कर्मचारी हैं, उनमें से कोई भी दावेदार की स्थिति के अनुसार स्वयं और परिवार के पात्र सदस्यों के लिए दावा पसंद कर सकता है।

दावा कौन पसंद करेगा, इसकी एक संयुक्त घोषणा प्रस्तुत की जानी चाहिए। यदि घोषणा प्रस्तुत नहीं की जाती है, तो पति की स्थिति के अनुसार पत्नी सहित सभी को रियायत का लाभ उठाना होगा।

यदि पति/पत्नी राज्य सरकार, रक्षा/रेलवे, निगमों या केंद्र या राज्य सरकार के स्थानीय निकायों और चिकित्सा सेवाएं प्रदान करने वाले निजी संगठनों द्वारा आंशिक या पूर्ण रूप से वित्तपोषित निकायों में कार्यरत हैं, तो चिकित्सा सुविधा का लाभ केवल एक ही स्थान पर लिया जा सकता है और एक संयुक्त घोषणा पत्र देना होगा। दावा कौन पसंद करेगा, इसकी जानकारी देना आवश्यक है।

यदि पति और पत्नी दोनों ईसीएचएस योजना में योगदान करते हैं, तो दोनों के माता-पिता पात्रता की अन्य शर्तों को पूरा करने पर चिकित्सा सुविधाओं के हकदार होंगे।
